Payment for Services:
- Proof of payment and photo identification are required.
- Fees are payable when services are rendered.
- We accept Cash, Check, Visa/MasterCard and Pre-approved PPO, EPO or POS Insurance where our physicians are preferred providers.
- We are not providers under any HMO (Health Maintenance Organization) plans.
- All fees and charges are based upon the time, amount, and nature of services.
- You are responsible for your own insurance benefits. Payment for services rendered to you and/or your dependents will not be delayed or withheld because of pending insurance claims.
- If current, accurate insurance and personal information is not provided, you will be required to pay for all charges incurred.

- We do not provide billing for vaccines (including school, travel, routine) unless your insurance has confirmed this is a covered benefit under your plan. A withholding deposit will be required if we are unable to verify benefits at the time of service.
- The adult that brings in a minor is responsible for payment at the time of service. Non-emergency treatment will be denied for unaccompanied minors unless prior payment arrangements have been made.
