What to bring to our office:

We accept most PPO insurance well as Medicare. Be sure to bring your current insurance card with you and notify our office staff if there has been a change in your insurance, phone number or address.

Our receptionists will ask to make a copy of your driver’s license for your chart. This is to assist our physicians with patient recognition during phone communications, when ordering medication refills, etc. We also require an accurate form of identification (usually your driver’s license) for some testing, such as screening for illicit drug use.

If you wear glasses or contact lenses and are here for a required physical examination, e.g., for a new job, or if you are being seen for an eye problem, please bring your corrective lenses with you. Patients being seen for a Department of Motor Vehicles (DMV) examination will not pass if they cannot demonstrate adequate vision.
